Human Timeouts

In an effort to balance things just bitrotting on our dashboards, we have the following policies when waiting for you:
- If an issue is tagged needs more info for more than 2 weeks, we will close it.
- If a pull request has been in the "Needs Revision" or "Changes Planned" state on Phabricator for over a week we will close it.
The data will remain in our database, so can easily be revived. If you find your contribution has been closed, first post on the issue and the on-call should notice. If they don't please open a new one referencing the old one.
